Good Faith Estimate Notice
You have the right to receive a Good Faith Estimate explaining how much your medical and mental health care may cost.
Under the No Surprises Act, health care providers are required to provide uninsured or self-pay individuals with an estimate of expected charges for scheduled health care services. This applies if you do not have insurance, are not using insurance, or choose to pay privately for services.
You may request a Good Faith Estimate before scheduling a service or at any time during your care. The estimate will include the expected cost of services that are reasonably anticipated based on your treatment needs at the time the estimate is created.
A Good Faith Estimate is not a contract and does not require you to receive services from Stock Health for Mental Balance. Actual charges may differ depending on your clinical needs, length of visit, diagnosis, services provided, frequency of appointments, or changes in your treatment plan.
To request a Good Faith Estimate, please contact Stock Health for Mental Balance through the patient portal or by calling the office.
